Registration and Commitment Form

By submitting this form I understand that I am making a commitment to 100 Women Who Care Annapolis Valley to make an annual donation of $400 – ($100 at each of four meetings between January and December) – given directly to local charities, non-profits and other worthy causes serving the Annapolis Valley (Hants and Kings Counties).

• I agree to fulfill my donation commitment even if I did not vote for the charity selected by majority vote.
• Memberships are based on a calendar year and renew in January.
• If I am NOT ABLE to attend a meeting, I will have another member or co-chair deliver my cheque in my place, OR I will honour my commitment within two weeks after each meeting.
